Athlete Contact Information
TVCC is using a new system for athlete paperwork collection and injury tracking software. We need your help in getting the information. Click on the link below and it will take you to a website ATS.
Fill out the Athlete information page as complete as possible(yellow cells are required). You can set the athlete username and password.
You will be directed to other pages, the insurance and contact information need to be filled out.
Select insurance: if you do not have insurance select No Primary Insurance, you still must select +add and select the payor as #1, and list None in yellow cell. If you have insurance fill out that page as complete as possible and you can upload a copy of your insurance card.
